Instructions for completing WEST Forwarding Services Power of Attorney (POA)
1. Enter your Federal Tax ID/EIN Number.
- If doing business as an individual, enter your Social Security number.
- If you’re a foreign importer of record and previously have been assigned a Customs Assigned Number (CAN) enter it here.
2. Check the box which applies to your company registration with the IRS.
- If executing POA as an individual, check individual.
3. Enter the full name of your company as registered with the IRS making sure to include dba only if registered with the IRS.
- If individual, enter your full legal name listed on your Social Security card and passport.
4. Enter the same business type on this line corresponding to the box you checked in #2.
5. For Corporations, enter the State in which your company is incorporated.
6. Enter your business address.
7. Enter the full name of your company to match what you entered in #3.
8. Physical signature and printed name (see #9). No electronic signatures, please.
9. Capacity of the person signing the form
- For Corporations, it must be signed by an officer of the Corporation such as President, Vice President, CEO, CFO, COO and authorized to execute documents.
- For Partnerships, any general partner can sign but a copy of the Partnership agreement showing all partners must also be provided. Partnership POAs are only valid for a period of 2 years.
Special Notes:
For Individuals, POA must also be accompanied by a copy of your Social Security Card and Passport. If Passport is not available, a valid driver’s license. Individual POAs also must be notarized by a Notary Public.
For Sole Proprietors, if using a Social Security Number, the above also applies. If using a Federal Tax ID number/EIN, provide a copy of your EIN assignment letter from the IRS.
For all types, if you have never been registered with US Customs as an importer, we will need a copy of your EIN assignment letter from the IRS. If you do not have it available, you can request one be faxed or emailed to you by contacting the below. Note this must be documentation from the IRS which indicates your company name, EIN # and be dated within the last 12 months.
You can call 1-800-829-0115 in order to obtain EIN verification (form 147C) if necessary. NOTE: W-9 applications, SS-4 applications and state documents, are unacceptable.
